This policy explains how we handle information from visitors to our public website and business inquiry forms.

PrimeVital LLC (“PrimeVital,” “we,” “us”) operates this public website to explain its services and respond to inquiries from healthcare organizations and other visitors. This policy applies to information we receive through this website, including its contact and clinic-discovery forms. Client engagements are governed by separate agreements and applicable law.
The separate Opportunity Navigator at access.theprimevital.com may have its own privacy terms; visitors should review those terms before using that tool.
2. Information we receive
When you submit an inquiry, we receive the name, organization, work email, and any phone number, message, or clinic-discovery answers you choose to provide. Our website’s hosting and security systems may also process ordinary connection information—such as IP address, browser details, requested pages, and request time—to deliver and protect the site.
Please do not send patient-identifying information or protected health information through public website forms or ordinary email. We ask prospective clients to use aggregate or de-identified practice information at this stage. If a prospective engagement needs patient-level information, an appropriate agreement and secure transfer process should be established first.
3. How we use information
We use website inquiry information to respond to requests, evaluate whether PrimeVital’s services may fit an organization, arrange follow-up conversations, maintain business records, and protect the website from misuse. Technical information is used to operate, secure, and troubleshoot the website. We do not use a website form submission as a request for medical care.
5. Retention, security, and your choices
We retain inquiry information for as long as reasonably needed to handle the request, maintain appropriate business records, or meet legal obligations. No internet transmission or storage system is completely secure. To ask about, correct, or request deletion of information you submitted through this website, contact us using the details below. We will respond as required by applicable law, subject to verification and lawful exceptions.
PrimeVital does not currently use additional website analytics, advertising pixels, or chat-based tracking for lead generation. The hosting and security services that deliver the site may use technical storage or cookies needed for those functions. If PrimeVital adds analytics or advertising tools later, this policy—and any required visitor choices or notices—must be updated before those tools are activated.
6. Patient information and privacy notices
PrimeVital supports healthcare practices under the practices’ clinical authority; it does not use this website to furnish or bill patients for care in its own provider capacity. Where PrimeVital handles protected health information on a practice’s behalf, it does so under a business associate agreement and the practice’s instructions, subject to applicable law. The treating practice’s Notice of Privacy Practices describes the patient-facing HIPAA rights and its uses of protected health information. Patients should contact their treating provider for that notice or for requests concerning their medical record. This public website does not provide clinical advice or an emergency communication channel.
